There are three steps in reporting a lost or stolen passport. Step 1. Complete Form DS-64, Statement Regarding a Lost or Stolen Passport. Complete Form DS-64 as best as you can, writing legibly and in black ink. If you do not know the answer to a question, try to provide an answer as best as you can. Step 2. Sign and Date Form DS-64, Statement Regarding a Lost or Stolen Passport. Form DS-64 must be signed and dated on page 1 of the form. Step 3. Submit Form DS-64 with a Completed Form DS-11, The Application For A U.S. Passport If you want your lost or stolen passport, submit a completed Form DS-11 and Form DS-64 in person to an Acceptance Facility or Passport Agency.
