Forms I-821D, I-765 and I-765WS should be filed concurrently (at the same time). Form I-821D is a brand new form specifically for childhood arrivals. Instructions for the form and the form itself can be found online at USCIS.gov.

File form I-765under category (c)(14), Deferred Action, or (c)(33), Consideration of Deferred Action. You will be required to submit the following documentation:

  • 2 passport style photographs taken within 30 days of filing
  • A Federal government issued identity document such as a passport (if available)
  • A copy of Form I-94, Arrival Departure Record (if available)

Form I-765 Worksheet will be used to establish your economic need for employment. You will be asked to provide some biographical information as well as financial information.
